Patient Rights and Responsibilities
Patient Rights:
B. Patients are not subjected to any treatment/procedure without voluntary, competent and knowledgeable consent, or that of a legally authorized representative. C. To assure that a patient is able to make a reasonably well-informed decision, they must be provided with a clear, concise explanation of their condition and be given all information regarding intended technical procedures.
D. Patients have the right to choose whether or not to participate in training programs, the gathering of data for research purposes, of other research and educational projects. E. Patients, or their legally authorized representative, shall have the right to refuse treatment to the extent permitted by law. When this refusal prevents the provision of appropriate care in accordance with Medica Forsyth’s standards and the standards of the health professional involved, the relationship with the patient may be terminated upon reasonable notice. F. It is the responsibility of all staff members to see that all patients are accorded their rights as outlined in patient’s bill of rights.
